We are excited that you are joining us! Here are a few things to help us get started: 

Transfers within the district: 

• Go to your previous school and formally withdraw your child. 
• Call the school to set up a tour! Both you and your child are welcome to attend. 
• Complete the Hope Horizon enrollment paperwork (this can be given to you in person or the files are below). 
• Complete the new client paperwork with TBD (this can be set up on the same day as your tour). 
• Fill out paperwork and drop medication at the clinic if needed. 
• Talk to us about transportation. 

New to the district: 

• Call or email Mrs. Jensen to discuss your child's previous placement. We will require copies of: 
  • Previous Individual Education Program
  • Previous Behavior Intervention Plan (if possible)
  • Previous transcripts
  • Shot records
• If Hope Horizon is an appropriate fit, we will schedule an annual IEP meeting as soon as possible to discuss commensurate services.
• After the IEP meeting, we will: 
  • Host a tour
  • Complete registration paperwork
  • Complete clinic paperwork and drop off medication
  • Discuss transportation
  • Complete intake meeting with the Lakeview team. 

Note: You are welcome to complete the following documents prior to registration or a registration packet will be provided during your tour.
